Extracurriculars and Passion Project?

I am looking at extremely prestigious schools and know in order for that I need extracurriculars and a passion project. Right now I'm in NHS, Key Club, Vice president of student gov, volleyball team captain my sophomore year (didn't do it junior year), gymnastics, varsity soccer, and I am a captain or "lead" at my job. I also go to a Bible camp every summer and get around 50 hours of community service so I will have about 200 hours of community service by my senior year. I want to go into architecture, civil engineering, (but have recently started liking the idea of dermatology) or something along those lines but I have no idea what kind of passion project I should do.
