Best way to submit transcripts on Common App?

Hi guys, I'm starting to feel a bit stressed out with gathering documents. Could someone explain how I'm supposed to submit my high school transcripts through the Common App? Is this something my counselor handles, or do I need to do something on my end? Thanks a ton!
