How do I ask my teachers for recommendation letters?

Hi everyone! I'm a junior, and I know it's time to start thinking about recommendation letters for college applications. I have a couple of teachers in mind, but I'm not sure how to approach them and ask for a letter. What's the best way to do this? Should I provide them with any specific information about myself?
