What is the common app essay format?

For the common app essay do I need a title even I already selected a prompt that common already gave us? I just need to copy and paste the essay from my docs to common app right? Do I need to include my name, major, and date for common app essay? Same goes to Covid 19 supplementary essay too, do I need a title for it? Thank you!

No titles, names, major, dates are necessary.

Just copy and paste into the Common App space.

If you add any additional words, they will count against the 650-word limit or other word limits.

If you have word space, you can always add a cool quirky fun title that has to do with your essay but please do not repeat the essay prompt as a title.
